steps To Schedule an evaluation
1. Click the name of the person you would like to schedule your evaluation with.
2. Click the type of evaluation you would like to schedule (DV or MH).
3. A calendar will appear with available dates, please select one date.
4. Click on our " Online Payment" page to pay for your appointment after booking.
5. Please fill out our Basic Information form https://forms.office.com/r/ZYY35vR3ms
Please Note:
- We utilize Zoom for all evaluations, sessions, and groups. Please make sure you are stationary in a private area alone with stable Wi-Fi.
- 24 hours notice is required for cancellations or you will receive a late cancellation fee.
- We do not permit refunds, all sales are final.
